This online form will assist you in notifying WorkCover of a work-related incident. Your obligations are outlined in the occupational health and safety legislation that WorkCover administers. To ensure you comply, you must refer to the appropriate legislation.
This form can only be completed by the employer, employer nominee, or person in control of the workplace where the incident happened.
This form must not be used to immediately notify 'serious incidents' (as specified in clause 344 of the OHS Regulation 2001). You must call 13 10 50 to immediately notify 'serious incidents' to WorkCover. This is a requirement under section 86 of the OHS Act 2000.
