You can complete an application form (Replacement of a WorkCover Construction Induction Card, form number ACC06) and submit to WorkCover with the associated fee of $30, together with proof of training details, identification and a signed statutory declaration

Valid identification will include a driver’s licence, passport, medicare card or birth certificate (minimum of two to be provided, one of which must be a photo-ID e.g. driver’s licence or passport). Payment may be made by cash (if lodging the application personally), cheque, money order or by providing credit card details.

All applications for replacements may be lodged with any WorkCover Office, or by posting to:

If these documents are being mailed, they will need to be certified by a Justice of the Peace.
