Proof of identity requirements

Applicants applying for a replacement of their Construction Induction Certificate are required to provide certified copies of their proof of identity (POI) documents when lodging by post or present their original documents to a WorkCover officer if applying in person.

Note that if another person is lodging the application on your behalf at a WorkCover office then certified copies must be provided.

POI requirements

Provide two certified copies of proof of identification and ensure that the JP number is specified on the certified copies (one type must be a photo).

• Drivers Licence
• Passport
• Medicare Card
• Birth certificate

Exceptions relating to school students

The following applies to school students if they cannot comply with the identification requirements set out above.

The following must be provided:

• Birth certificate

And one other form of proof which includes:

A current student identity card or a certificate or statement of enrolment up to 2 years old from an education institution. These could include:

• Student identity card
• Student library card
• Student bus pass
• Current credit card or account card from a banking institution or passbook
• Mobile phone account in their name which is less than one year old
